O: Removed wing wrap from L wing. Blood strike through on bandage on ventral aspect of metacarpus but no active bleeding seen when bandage removed. Significant bruising present on metacarpus. IM pin in place and fracture site feels stable. Performed gentle PT on wing. Covered sutures with TAO, telfa, tegaderm, placed fabric wrap on wing. Sutures holding over the L femur but skin looks inflamed and there is a small gap in the suture line. Covered with TAO, telfa/tegaderm.

O: See surgery report for details. Placed IM pin in the L metacarapal fracture, closed wound on ventral metacarpus and wound along lateral femur with sutures. Covered sutures on L metacarpus with TAO, telfa/tegaderm and placed figure-8 wrap on wing.

A: Bone looked viable during surgery, able to stabilize and align fracture with IM pin.

Surgery report
Procedure - Fracture repair - metacarpus
Performed by - AD
Surgery time - 30 minutes

Pre-medicated with torb IM.  Anesthestized via isoflurane facemask
with 2.5 uncuffed ET tube.  Placed bird in dorsal recumbency
and plucked feathers over L metacarpus and scrubbed site for
surgery.  Extended open wound with #15 blade, dissected down
to fracture ends.  Bone appeared in better alignment than on
intake x-rays, bone appears viable.  Placed IM pin up the proximal
fragment and out the carpus, blunted the other end of the pin
and drove across the fracture site.  Took x-rays to confirm placement.
 Flushed wound site and closed with 5-0 webcryl in horizontal
mattress pattern.  Cleaned wound on L femur, closed with 5-0
webcryl with simple continous pattern.  Turned bird onto sternal
recumbency and bent IM pin around the dorsal aspect of the bones.
 Covered ventral sutures with TAO, telfa and tegaderm, placed
figure-8 wrap on wing.  Bird recovered uneventfully from anesthesia.

S: Standing for catch.

O: Removed large fabric wrap from L wing. Changed bandage over ventral L metacarpus - no bone seen, open wound present. Crepitus at fracture site. Cleaned wound and covered with duoderm gel, telfa/tegaderm, placed splint back on fracture and placed figure-8 wrap on wing. New wound found over the lateral aspect of the L femur, cleaned and covered with duoderm gel/telfa/tegaderm. Large retinal tear and debris in vitreous along the ventral aspect of the L eye, but bird still has menace and PLRs OU, menace may be slightly reduced on the L compared to the R but bird still has some visual reaction.

A: Bone appears to remain viable as per KK, but coloration appears darker compared to intake. Will see how rest of bone looks tomorrow during surgery. Fabric wrap and splint combo appears to be keeping metacarpals stable. DNE food from yesterday, so will hand feed this evening in preparation for surgery.
Weight change: +19 g (5 %)

S: Jumped into ceiling when door opened. Flared. Footy.

O: Removed fabric wrap and bandage. Noted a good amount of bloody strikethrough on bandage. Moist hematoma forming over wound site. Gently cleaned with dilute chlorhex- did not removed hematoma to get visual on the bone. Packed with a generous amount of duoderm + a small amount of manuka. Bandaged with telfa/tega. Replaced splint and secured with paper tape wrapped gently around carpus. Replaced fabric wrap. Gave meds PO. Did not remove any scraps from cage in case he wants to eat overnight.

A: BAR. Mentation very good. The fact that this wound appears to be bleeding, helping to keep the exposed bone moist, makes me cautiously optimistic. I'd advise against removing any hematoma or scab formation over the wound. Our best chance of keeping the bone viable with be to continue hydrating that wound with BID bandage changes until the bird can go to Sx.

O: Collected fecal. Collected blood from right medial metatarsal. Performed IOP study. Removed L figure 8 wrap and metacarpal bandage. Bandage soaked through. Cleaned L metacarpal wound w/ chlorohex and packed w/ duoderm gel. Covered w/ telfa/tegaderm. Placed L fabric wrap w/ metacarpal splint. Applied tail guard. Sprayed w/ Adams. Gave fluids and oral meds. Dropped 60g of chick.

A: Very guarded prognosis as bone that is exposed over L metacarpal wound is already becoming discolored. Will change bandage BID to keep as moist as possible. L fabric wrap w/ metacarpal splint should provide enough support to keep metacarpal stabilized.

P: BID bandage changes. Monitor L fabric wrap/carpal splint. End care if bone coloration worsens.

O: Checked vitals. Gave torb. Performed intake exam. Took rads. Cleaned L ventral metacarpal wound and tucked metacarpal bone back under hole. Packed wound w/ duoderm gel and covered w/ telfa/tegaderm. Applied L figure 8 wrap. Gave fluids and oral meds. Placed in cage in ICU.

Initial treatment: 0.08 cc Torb; 0.26cc Meloxicam; 0.08 cc Baytril, 1/4 tablet Clavamox; 0.58cc Calcium Slurry.

A: Patient suffering from L metacarpal fracture. Compound wound is present on ventral aspect of fracture site and bone of metacarpus major was exposed. Was able to get bone back under skin and packed with ointment. Guarded prognosis as bone was exposed for unknown amount of time and will need surgical repair asap. Patient is otherwise healthy and vital signs are all stable.

P: Rx meloxicam, Clavamox, baytril, tramadol, calcium slurry, pentoxifylline and fluids. Monitor L figure 8 wrap. SID bandage changes and pack metacarpal wound w/ duoderm gel. Surgery 5/19. Collect blood and fecal 5/17. Take IOPs 5/17. SF in AM, HF in PM if DNE starting 5/18.
